When do I need a fit note?
7 days off sick or less
If you're off work sick for 7 days or less, your employer should not ask for medical evidence that you've been ill. Instead they can ask you to confirm that you've been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification.
Find the self-certification form here.
More than 7 days off sick
If you're off work sick for more than 7 days, your employer will usually ask for a fit note (or Statement of Fitness for Work) from a GP or hospital doctor. Fit notes are sometimes referred to as medical statements or a doctor's note.

Requesting a Fit/Sick Note
To request a sick note continuation please
GP Letter / Medical Certificates
For all other private administration requests such as GP letters, private medical certificates, insurance reports etc please hand in your request to our reception team. Your form/request will be reviewed, and you will be advised of the fee for completion of the work.
Please note the following information before submitting your request to the admin team:
- Pre-payment may be required prior to completion of Non-NHS and Private work. This will be communicated to you once a request has been received by the Practice.
- Prior to commencement of any Private work, a cost will be communicated to you. However, prices may vary following completion depending on the complexity of the request.
- You will be notified when your request is complete and available for collection.
- It is at the GPs discretion whether they complete private work, we are not contractually obliged to accept your request.
- While the Practice aims to complete work within 28 working days, this timeframe is not guaranteed. Due to increasing demands for Private and Non-NHS work, you may experience prolonged wait times. Please consider this if you are under strict time frames for work to be completed.
- For any requests that require an appointment with a General Practitioner, please be aware that waiting times may vary. If you have a specific deadline by which your medical needs to be completed, we kindly ask that you inform the Practice as early as possible. This will allow us to assess whether we can accommodate your request. If we are unable to meet your deadline, we will advise you further if an alternative service is available.
