The World Health Organisation defines health as "a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity." At Hartlepool & Stockton Health we are leading a pilot to provide extra, holistic support to patients who are living with long term clinical conditions which may put them at risk of frequent, unplanned hospital admissions.
Often known as “Social Prescribing”, our Care Coordination service helps patients to focus on the important social and mental factors that affect their health. Our 20 Care Coordinators work with every surgery in Hartlepool and Stockton, helping over 100 patients each week to create their own simple plan to enable them to feel better. We work with over 40 community and voluntary sector partners to help local people to stay safely and happily in their own homes.
In a recent survey 27% of patients said that they felt the service was life changing and 69% felt their circumstances had vastly improved.
